Subject: [PATCH] Spawn SSH directly on startup instead of a local shell first

The old update-right-status auto-attach flashed a local cmd.exe tab
first and only attached the SSH domain after a delay once the mux
had registered it, so startup never actually connected immediately.
Now gui-startup spawns the first configured SSH domain's `ssh ...`
command directly as the window's first program, so there's no local
shell tab and no attach-timing race to work around.
